How to Complete Your Subscription Documents
When you invest, one of the final sections is to complete your subscription documents. This ensures we have all the correct details to process your investment and issue tax documents like your annual K-1.
At the top of your form, you’ll first select your account type by checking one of these boxes:
- Individual / Joint
- SD-IRA
- Entity
- Solo 401k
- Trust
Once you select your account type, you’ll fill out only the section that matches it.
This article provides clear guidance for how to complete the form for Individual/Joint accounts, SDIRA, Solo 401k, and Trust accounts.
For Individual & Joint Accounts
If you’re investing in your personal name, or jointly with a spouse or partner, complete the Individual & Joint Accounts section.
What to fill in:
Primary Investor Name & SSID / Tax ID
- Your full legal name as it appears on your tax return.
- Include your Social Security Number (SSN) or Tax Identification Number (TIN).
- Make sure this is accurate to avoid processing delays.
Secondary Investor Name & SSID / Tax ID
- If this is a joint account, list the full legal name and SSN/TIN of the second investor (such as your spouse).
- If it’s an individual account, leave this blank.
Address for Tax Return
- The address to be used for all tax documents, including K-1s.
- Typically this is the address on your personal tax filings.
For SDIRA Accounts
If you’re investing through a Self-Directed IRA (SDIRA), complete the SD-IRA Accounts section.
What to fill in:
Investor/Entity Name
- The formal name of your IRA account as provided by your custodian.
- Example:
XYZ Trust Company FBO John Smith IRA
Investor/Entity Address
- Often your personal mailing address, unless your custodian instructs otherwise.
- Confirm with your custodian for the preferred address.
Custodian Name
- The name of the custodian that manages your SDIRA, such as Equity Trust, Strata Trust, Advanta IRA, etc.
Custodian Address & Phone Number
- The custodian’s official mailing address and phone number.
Account Title Provided by Custodian
- Exactly how your custodian titles your account on official statements.
- This is important for processing and matching records.
Address for Tax Return
- Typically your address. Some custodians prefer to receive tax documents directly. Ask your custodian what to list.
EIN Provided by Custodian
- Most SDIRAs do not have a separate EIN.
- If your custodian hasn’t issued an EIN, leave this blank or write “N/A.”
For Solo 401k Accounts
If you’re investing through a Solo 401k, complete the Solo 401k Account section.
What to fill in:
Investor/Entity Name
- The formal name of your Solo 401k plan, often structured with your business name.
- Example:
ABC Consulting LLC Solo 401k Plan
Investor/Entity Address
- Your business or plan mailing address.
Account Title
- As specified in your Solo 401k plan documents.
Address for Tax Return
- Where tax documents for the plan should be sent.
Tax ID (SSID or EIN)
- This is often your business’s EIN tied to the plan.
- In some Solo 401k setups it might be your SSN if it’s structured that way.
- Check your Solo 401k plan documents or with your plan administrator to confirm.
For Trust Accounts
If you’re investing through a Trust, complete the Trust Accounts section.
What to fill in:
Legal Name of Trust
- The name exactly as it appears on your trust documents.
Name(s) of Trustees
- List all trustees authorized to act on behalf of the trust.
Name(s) of Beneficial Owner(s)
- Individuals who ultimately benefit from the trust.
Entity EIN of Trust or Primary SSID
- Enter the trust’s EIN. If the trust doesn’t have one and files under a trustee’s SSN, use that.
Entity Address for Tax Return
- The mailing address used by the trust for tax filings.
Additional Trust Tax Reporting Questions
The form also asks:

Does the trust listed in your investor agreements file a 1041 tax return?
- If yes, reconfirm the tax ID for the trust return.
- If no, you’ll need to provide the name and SSN of the beneficial owner who reports this income on their personal return.
Key Notes for All Investors
- Make sure all names and tax IDs exactly match your official documents (personal tax return, custodian records, or trust paperwork).
- This ensures your investment is processed smoothly and your K-1s are prepared correctly.
